Child’s Proof of Immunization
Child’s Proof of Immunization and Health Screenings
All Inspire Connections Academy students must provide evidence of their immunizations on a State of Idaho Official Immunization Form or a booklet obtained from your physician with each dose signed/stamped.
Not Sure What Immunizations Are Required?
According to the Idaho Department of Health and Welfare (IDHW), "to enter or transfer into a public or private school, all children in preschool and grades K–12 must meet immunization requirements outlined below at registration and before attendance. No child shall attend school without proof of immunization status".
For a current list of the "Minimum Immunization Requirements for School Entry", visit the School Immunizations Requirements section of the IDHW website.
Where Can I Get Immunizations for My Student?
Immunizations can be obtained from doctor’s offices, clinics, or hospitals. Pediatricians and family doctors or their nurses or medical assistants can give your student the shots he or she needs to meet the requirements for school enrollment. In addition, some large chain pharmacies offer immunizations for older children and adults.
What if I Need Assistance Paying for Vaccines?
If you need assistance paying for shots/vaccinations, you may be able to get free or reduced immunizations through this program:
- Vaccines for Children (VFC) program is a federally funded program that provides vaccines at no cost to children who might not otherwise be vaccinated because of inability to pay. Children who are eligible for VFC vaccines are entitled to receive pediatric vaccines that are recommended by the Advisory Committee on Immunization Practices.
Immunization Exemptions
Idaho law allows a parent/guardian to claim an exemption from immunization requirements for his or her child for medical or philosophical reasons. All exemptions must be documented on the official Idaho Certificate of Immunization Exemption(opens in a new tab) form. Letters must be submitted each year.
- Philosophical Exemption: Parents/guardians are required to write a comment of explanation regarding their beliefs on the exemption form.
- Medical Exemption: A medical exemption requires the signature of a licensed physician and certification that the child has a medical condition which prevents him or her from receiving required vaccinations.
